Full-Time Case Manager

Position Overview

This position is for a full-time Case Manager assisting trafficking survivors in the Denver Metro area. Working in a fast-paced environment with an all-female clientele base from our Care and Education Center in Parker as well as fieldwork in the Denver Metro area. This person will also be taking incoming calls, intake and care coordination.

 The Case Manager coordinates services to survivors of trafficking, writes case plans, and assists clients with tasks if needed, or delegates tasks to milieu staff and interns.

Primary Duties

●  Managing workflows through established systems, including meticulous record-keeping, efficient task prioritization, and the ability to implement and maintain systems put in place

●  Collaborate with clients to identify achievable goals and objectives that address relevant issues such as health, housing, income, employment, education, and legal needs

●  Develop and implement an individualized case plan for clients and monitor progress toward case management goals

●  Meet weekly with each client to develop weekly case plan, and discuss their goals and progress

●  Work with the Program Director on coordination of care and case plans, client appointments and weekly client task lists

●  Work with crisis clients to determine their next steps or assist with resources

●  Maintain appropriate boundaries with participants, observing all confidentiality and HIPAA protocols

●  Utilizes different case management modalities (i.e. Motivational interviewing)

●  Provides services to clients in a trauma-informed, culturally competent manner

● Work in a fast-paced environment while handling many details

● Update staff weekly on client updates or announcements

●  Building resources and connections in the community

● Work with interns on resources and outreach

●  Act with utmost professionalism, responding promptly and respectfully to all inquiries, suggestions, requests, and complaints in accordance with agency policy

●  Maintain excellent written records and required statistics, documentation and administrative duties

 

Qualifications

●  2 years’ experience in the field and CAT preferred. (Completion of CAT in 2 years)

●  Bachelor’s degree in psychology, Human Services, Social Services, or related field preferred 

●  Excellent verbal and written communication skills, including detail-minded record-keeping

●  Proficient with Microsoft Office, spreadsheets, and Google Suite knowledge

●  Ability to apply positive engagement, motivational, and leadership skills in a culturally appropriate manner, working with adults from diverse and challenged backgrounds

●  Be able to work independently as well as part of a team, with good personal boundaries and appropriate use of authority

●  Ability to hear and process critical feedback, as well as provide it to others

● Ability to meet deadlines and follow through on commitments.

●  Experience working with justice-system programs helpful

● If offered a position, must complete a criminal background check

●  Must have a valid Colorado Driver's license and a recent clean driving record. The ability to transport the people we serve in your vehicle. Safe, dependable vehicle required

● Must have and maintain a current Colorado auto insurance policy.

●  Proof of automobile insurance and clean background check required

●  Some travel within Denver Metro area

●  Evening and some weekend work may be required

●  Knowledge of Covered policies and procedures and ability to enforce them

● Excellent rapport with the client population

●  Demonstrated ability to constructively work with people in crisis

 

Time Requirements

This position will be completed in 40 hours per week.  This position is mainly Monday-Friday during business hours but may at times include evening, weekend or holiday hours.

Work Environment

This job operates in a professional office environment as well as fieldwork and uses standard office equipment such as laptop computers, desktop computers and telephones.
