Case Management Assistant

Position Overview
This non-exempt position is for a full time Case Management Assistant working with an all-female clientele base in a residential house setting in Aurora, working from our offices in Parker and remote fieldwork assisting trafficking survivors in the Denver metro area.
As the Case Management Assistant, you will work closely with the Executive Director developing and implementing individual case plans and assist in coordinating services and resources for clients. Managing client files, scheduling, and transporting clients to appointments, communicating with clients and partner organizations, and ensuring documentation accuracy. Your role will contribute to the efficient functioning of the case management team and the delivery of high-quality services to clients in a trauma-informed environment.
Primary Duties
Work with the Executive Director to develop individual case plans and maintain all documentation with the program team.
Help clients identify achievable goals and objectives for their individual case plans that address all relevant issues: income, employment, education, health, mental health treatment, substance abuse treatment, housing, recovery, life skills, and legal needs
Collaborate with clients to develop, implement, and revise a 6–8-month individualized case plan and monitor progress toward case management goals
Meet at least weekly with each client to develop weekly case plan and discuss their goals and progress
Act with utmost professionalism, responding promptly and respectfully to all inquiries, suggestions, requests, and complaints in accordance with agency policy
Communicate with staff, volunteers, and partner agencies in a respectful, professional manner
Maintain appropriate boundaries with clients, observing all confidentiality and HIPAA protocols
Maintain excellent written records and required statistical forms, as well as administrative paperwork such as mileage sheets, expense reports, and disbursement requests.
Review documentation for completeness and compliance with organizational policies and regulatory requirements. Maintain database records for clients in a timely and accurate manner
Participate in regularly scheduled supervision, and utilize supervision/consultation as needed
Utilizes different clinical case management modalities (Motivational interviewing, coaching, intensive case management, active listening, etc.)
Provides services to clients in a trauma-informed, culturally competent manner
Take incoming calls, intake assessments, care coordination
Schedule appointments and follow-up visits for clients, coordinating with case managers and other team members. Transport clients to appointments or coordinate transportation with milieu staff
Identify areas for process improvement and contribute to the implementation of best practices within the case management program
Occasionally filling in for residential staff as needed to cover house shift needs
Qualifications:
High school diploma or equivalent; a degree or college coursework in social work, psychology, or a related field preferred
Previous experience in an administrative support role, preferably in a social services or related field
Excellent communication skills, both verbal and written, with the ability to interact effectively with clients and team members
Excellent computer skills, working with Microsoft, Google Suite, client database, and other technology products
Knowledge of confidentiality regulations and ethical standards related to client information
Ability to work independently as well as part of a collaborative team
Ability to apply positive engagement, motivational, and leadership skills in a culturally appropriate manner, working with adults from diverse and challenged backgrounds
Be able to work independently as well as part of a team, with good personal boundaries and appropriate use of authority
Ability to hear and process critical feedback, as well as provide it to others
Ability to meet deadlines and follow through on commitments
Other Requirements

Must have a valid Colorado Driver’s license and a recent clean driving record
Ability to transport the people we serve in your vehicle. Safe, dependable vehicle required with a current vehicle registration maintained
Must have and maintain a current Colorado auto insurance policy
Excellent rapport and a deep understanding of client population
Confidence in the mission and compassion for assisting survivors of trafficking and their ability to heal and become self-sufficient
Some travel (within Aurora and the Denver Metro area)
Evening and some weekend work may be required as needed
Knowledge of Covered policies and procedures and ability to enforce them
Demonstrated ability to constructively work with people in crisis
Time Requirements
This position will be completed in 40 hours per week. This position may include evening and weekend hours as needed but primary hours will be standard business hours M-F 9-5.
Work Environment
This job operates in a professional office environment as well as fieldwork and uses standard office equipment such as laptop computers, desktop computers and telephones.
